The New York State Donate LifeSM Registry for Organ and Tissue Donations
The NYS Donate LifeSM registry, a new donor registry to begin in July 2008, includes only individuals who have signed a consent certification to be organ and tissue donors upon their death. Family consent is not required for donors who have already given their signed legal consent for donation.
How Can I Become an Organ and Tissue Donor?
There are three ways to enroll in this registry:
How can I have the words "ORGAN DONOR" displayed on my DMV photo document?
If you enroll through DMV in person at a DMV office or by mail, your driver license or non-driver ID card will display the words "ORGAN DONOR." If you enroll through the DOH or a donation organization, your DMV document will not have the words "ORGAN DONOR." displayed on it. If you enroll in the registry, you are enrolled regardless of whether or not the words "ORGAN DONOR." are displayed on your DMV document.
To change your enrollment, you must contact the DOH. If you change your enrollment with DOH, the change will not change your DMV photo document.

How Can I Make a Monetary Contribution to Aid Organ and Tissue Donation?
When you apply for a driver license original, duplicate, amendment or renewal, you can make a voluntary $1 contribution to the "Life…Pass It On"" Trust Fund. Contributions to the Fund are used for organ donation and transplant research and education projects that promote organ and tissue donations.
